The Patient-Centered Medical Home Without Walls: Clinicians and Community Health Workers Partner to Provide Care for Frail Older Adults—The CARIÑOS Model

Resumen

Two major challenges led to our team’s developing the CARINOS Model. The first was the limitations in the care we experienced after becoming a National Committee for Quality Assurance (NCQA)-recognized Patient-Centered Medical Home (PCMH) and Geriatric Care Medical Neighborhood. The second, and the more pressing challenge, was the inability to see our patients in the clinic due to the COVID-19 Pandemic. These two challenges are described in detail below.
