Summary form only given. The author reviews the biological bases of the EEG. The relationships of alterations in recordings with deteriorating cellular metabolic environment and drug management are discussed. Applications of EEG monitoring are listed in which the EEG is used as an index of neuronal viability, and in which the surgical, anesthetic or medical management is altered to improve outcome. The relationship between altered EEG, the ischemic penumbra and irreversible damage is also reviewed.
